Under the heavy influence of his Mad Enhancement, he has lost most of his reason and speech, behaving impulsively like a beast with sadistic combat urges toward enemies. However, his profound affection for Rome and his niece, Nero, persists in his subconscious, and bringing them up can occasionally stay his rage. When his sanity is intact, he displays a rational, brave, and deeply compassionate character.

He fights primarily in unarmored melee combat, dealing devastating damage through boxing-style punches, dropkicks, headbutts, and claw strikes. His Noble Phantasm, Flucticulus Diana, projects moonlight to broadcast his lunacy across a wide area, causing mental contamination that seals enemy skills and Noble Phantasms. He also possesses Imperial Privilege to temporarily borrow diverse skills, Sadistic Constitution to heighten offensive aggressiveness, and a technique that boosts his physical power at the cost of taking self-inflicted damage.

In first-century Rome, he was initially cherished by the populace as an energetic and reasonable monarch. After recovering from a severe illness two years into his reign, he underwent a drastic shift into madness and governed through fear, vice, and tyranny for four years before being assassinated. Roman tradition ascribed his sudden transformation to being loved by the moon goddess Diana. As a Servant, he harbors a quiet wish to be cured of his lunacy and restored to the wise ruler he once was.
Relationships
Nero Claudius is his beloved niece, whom he dotes on completely even while consumed by insanity. He also cherishes his sister, Julia Agrippina. He holds supreme reverence for Romulus, the founder of Rome, whose presence calms his violent outbursts. Gaius Julius Caesar is his ancestor. Toward his Master, Fujimaru Ritsuka, he exhibits a reassuringly paternal demeanor.
